Experience You
Can Count On

Our clinicians are experienced, well-trained, and know that the team approach to your medical care is the best way to help you reach your health care goals and to help you avoid going back to the hospital. That is why we conduct an in-depth initial assessment with you, and hold team conferences to review and improve the care you are receiving. We stay in touch with your doctor, and counsel and educate you and your family about what is happening with you medically and how you can help your condition.

Patient Care Advocates-Home Health Care Services

Patient Care Advocates is a superior home health company. Our emphasis on compassionate, high quality care, and superb care coordination means we have some of the very best clinical outcomes in the United States. It is a reason we are a preferred provider in Tucson medical centers and co. It is also the reason our client survey results (both those taken by Medicare's survey company and our surveys) are always excellent.

Tucson Medicare Home Health Care Caregiver
We provide care for a wide range of medical conditions. Some examples are diabetes, diseases, infections, care needed as a result of surgery, injuries, and wound care. We also help you with your medications, regularly re-evaluate your medical needs, and provide you with preventative and rehabilitative nursing and therapy. Our team of experts is comprised of Physical Therapists, Registered Nurses, Occupational Therapists, Social Workers, Certified Nursing Assistants and Speech Language Pathologists.

What we do for our clients?

Registered Nurses conduct in-depth medical assessments of each of our patients so that we can insure all nursing and medical care is tailored to meet patient needs.

Physical Therapists help patients with their rehabilitative needs, range of motion, balance, strength, stability, and endurance. They also help with home safety.

Occupational Therapists help patients regain their ability to care for themselves, increase their motor skills and help patients with cognition (reasoning and judgement).

Social Workers help clients cope with issues in their daily lives. They also help clients find needed community resources such as financial assistance and meal service.

Speech Pathologists help with swallowing difficulties, speech problems, voice disorders, and non-vocal communication device training.

Home Health Aids provide hands-on personal care needed to facilitate and/or maintain or prevent deterioration of a patient's health. They also help with: bathing, dressing, grooming, foot care and ear care, feeding assistance, colostomy care, simple dressing changes, light house keeping and more.

We also provide private caregiver services
and advocacy services.
